HEALTH INSURANCE: As of September 1, 2018 The Department of Labor came out with their final ruling on Association Health Plans. This ruling allows existing Associations to pool together members and member groups utilizing economies of scale by spreading the risk to buy health insurance through the association.

Current Insurance Carriers participating:
  • Blue Cross/Blue Shield of Alabama
  • United Healthcare

If we are able to achieve a large enough risk pool (not including spouses, dependents), the large pool size could enable the following potential benefits of these new rules and regulations:
  • Potentially significant lower premiums
  • Potentially significant lower out of pocket costs
  • Potentially lower co-pays
  • Potentially lower prescriptions costs

It is imperative that you complete the census if you would like to at least explore these plans and rates. The census consists of only 5 items to submit: Name, Birthdate, Zip Code, Gender, and Preferred Coverage Tier.

The census has been discontinued.

If family members will also use the policy, give us the same information for each one by filling out the form for each family member.

If you have already entered the census through a previous email to AFOA, please submit this form. It collects a couple more pieces of information the previous census did not that the insurance company needs to form a proposal.
